Reducing Rates of Chronic Disease
Chronic diseases are affecting more Americans — at younger ages and at a great financial and societal cost. How can state policymakers help prevent these conditions and help effectively treat those who are already impacted?

Presenters:

 Mr. John Miall; Special Consultant, American Pharmacists Association Foundation’s Asheville Project

 
Discussion: What is your state doing to help reduce the financial and societal cost of chronic disease? Each state’s delegates will have the opportunity to share their work.

Addressing Health Disparities
Data show that racial and ethnic minorities are more likely to struggle with certain health issues. Presenters will explore the state of health disparities today: Where are we succeeding? Where could we improve? How can we promote cultural competence?
